License Fees

If this is a renewal and a new metal tag is needed, please request a new metal tag with your pet's renewal. Otherwise, a receipt only will be sent.

Required Paperwork

• A current rabies vaccination certificate is required if the rabies vaccination on file has expired. Please check your certificate for correct address and pet information.

Your paperwork will not be returned to you, so please only send copies and retain the originals for your records.

Mailing Address

To license by mail:

1. Enclose copies of all required paperwork.
2. Include $20 for each of your pets.
3. Make your check or money order payable to "Manatee County Government." Please do not send cash.
4. Mail your paperwork and payment to:

License Certificate

License sales may be reported with triplicate certificates which are provided free of charge or with a stamped rabies certificate. The PDF below may also be completed and printed out. If using the PDF, please be sure to print three copies: one for the pet owner, one for the clinic's records, and one for PetData. Please do not save and email the completed forms, as the data tends to be lost in the process.
