- Proof of spay/neuter, such as a signed statement from a veterinarian or an invoice for services is required if the pet has been altered since the last licensing period and you are applying for the lower spayed/neutered license fee. A Spay/Neuter Statement is also accepted as proof of spay or neuter provided that the statement is signed by the owner and the date of surgery and clinic which performed the surgery is included.
- Proof of age is required to receive the discounted fees for senior citizens age 62 and over.
- Proof of disability is required to receive the discounted fee for disabled citizens. Proof of disability must be submitted annually. An Eligibility Form is accepted as proof of disability provided that the statement is signed by the owner or the owner's legal guardian. A photocopy of any of the items listed on the form is also accepted as proof.
