Update Vaccination

Your pet's rabies vaccination must be current throughout the license term for the license to be maintained as current. If your pet is re-vaccinated mid-license term, please submit a copy of the updated rabies vaccination certificate.

Check the certificate for correct owner and pet information. Write in any corrections as needed. Also write "Update rabies on current license" and your pet's license tag at the top.
