Required Paperwork
Required for All Pets:
- Current rabies vaccination certificate: A current
rabies vaccination is required in order to license your pet. Unless
we have a current vaccination certificate already on file, a current
certificate must be submitted. Please check your certificate for correct
address and pet information.
Required for Discounted License Fees:
- Proof of Spay or Neuter is required if you are applying
for the lower spayed/neutered license fee. Proof can be a signed statement
from a veterinarian or an invoice for services. Once this proof is
on file, it will not be required for each license renewal.
- Proof of age is required to receive the discounted
fees for senior citizens age 65 and over. A photocopy of any document
showing the owner’s name and date of birth, such as a driver’s
license or ID card, is acceptable. Any personal or sensitive identification
numbers may be removed from the copy prior to submission.
- Proof of disability status is required to receive
the discounted fees for disabled persons. A photocopy of any document
showing that the pet owner receives disability benefits under Social
Security or an equivalent disability retirement program is acceptable
to meet this requirement.
- Proof of registered service animal is required for
a new service dog license. The owner of a service animal must provide
documentation of the animal being registered with a service animal
registry. Service dogs must be actually being used to act as a guide
for the blind or deaf in order to qualify for the discount.
Note: Please send legible copies of your paperwork and maintain a copy
